Abstract Background A metaplasia-dysplasia–carcinoma sequence is the most plausible carcinogenic pathway for gallbladder cancer. Although incidence of carcinoma increasing, little known about its precancerous lesions. The aim this study was to determine temporal changes in prevalence low-grade dysplasia (LGD), high-grade (HGD) and adenocarcinoma associated risk factors. Methods We retrospectively identified consecutive patients who underwent cholecystectomy between January 2011 March 2020. Patients were grouped according histology: no dysplasia; LGD; HGD; adenocarcinoma. Fitted linear models estimated trends mean age all histological outcomes. Logistic regression Results total 5 835 included analysis. LGD 1.47%, HGD 0.17% 0.19%. Prevalence diseases increased over time, at diagnoses decreased time. In a multivariate logistic model, with as reference group, female sex odds (OR 4.57, 95% CI 3.07-10.10, p = <0.0001). BMI not disease risk. Conclusions Our data suggests lesions are increasing younger patients. factor cholelithiasis, progression. If occurring dysplasia-carcinoma sequence, progression period 20 years. Further research required explain both significant disparity potential environmental factors dysplasia.
